- You may download and prepare the “New Patient Registration Form“, and bring in for your appointment
- What to bring on your first appointment:
- A Valid Medicare card
- Photo ID
- Lists of medical conditions
- List of medications
- Latest pathology reports, imaging reports if it is possible
- For children’s immunization, please bring in the blue book
